
Care Coordination is an essential step in accessing services through the New York State Office for People With Developmental Disabilities (OPWDD). It ensures that individuals receive personalized support and services that align with their goals, preferences, and needs.
At Tailored Independence LLC, we guide individuals and families through the Care Coordination process, ensuring they have the necessary support to access Self-Direction and other OPWDD services.
Steps in the Care Coordination Process
1. Choose a Care Coordination Organization (CCO)
✅ Select a Care Coordination Organization (CCO) that serves your region.
✅ CCOs provide Care Managers who help individuals develop a service plan.
✅ Examples of CCOs in New York include Advance Care Alliance (ACA), LIFEPlan, and Person Centered Services.
2. Enroll in Care Management
✅ Choose between Basic Care Management (limited support) or Health Home Care Management (comprehensive support, including medical and behavioral health coordination).
✅ Your Care Manager will assist with navigating OPWDD services and ensuring access to Medicaid-funded supports.
3. Develop a Life Plan
✅ Work with your Care Manager to create a Life Plan—a person-centered document outlining:
Your strengths, needs, and goals
The services and supports you will receive
How these services will help you achieve greater independence and quality of life
✅ The Life Plan is reviewed and updated regularly to ensure it continues to meet your needs.
4. Apply for HCBS Waiver Services
✅ If not already enrolled, your Care Manager will help you apply for the HCBS Waiver, which funds OPWDD services like Self-Direction, Community Habilitation, and Respite.
✅ Once approved, you can begin receiving OPWDD-funded services.
5. Choose Self-Direction or Traditional Services
✅ Decide if you want Self-Direction, which provides more control over your budget and service choices, or Traditional OPWDD services.
✅ If choosing Self-Direction, your Care Manager will help connect you with a Self-Direction Support Broker (like us!) to develop a budget and service plan.
6. Ongoing Support & Service Coordination
✅ Your Care Manager will monitor and adjust your services as needed.
✅ They will work with you, your Support Broker, and other service providers to ensure your needs are met.
✅ Regular Life Plan reviews ensure your services continue to align with your goals.
