Understanding the Front Door Process

The Front Door process is the first step to accessing services through the New York State Office for People With Developmental Disabilities (OPWDD). It is designed to ensure that individuals with disabilities receive person-centered services that support their goals for independence, inclusion, and personal development.

At Tailored Independence LLC, we help individuals and families navigate the Front Door process, making it easier to access Self-Direction and other OPWDD services

Steps in the Front Door Process

1. Eligibility Determination

✅ Submit documentation to OPWDD to determine eligibility for services.
✅ This includes medical records, psychological evaluations, and IQ/adaptive assessments.
✅ OPWDD reviews the information and provides an eligibility decision.

2. Front Door Information Session

✅ Attend a mandatory Front Door session (offered virtually or in person by OPWDD).
✅ Learn about available services and supports, including Self-Direction.
✅ This step is required before moving forward in the process.

3. Service Authorization & Care Coordination Enrollment

✅ Enroll with a Care Coordination Organization (CCO), which will assign a Care Manager.
✅ The Care Manager will help you develop a Life Plan and apply for services.
✅ Choose Self-Direction or Traditional Services, based on your preferences.

4. Self-Direction Enrollment (If Chosen)

✅ Work with a Self-Direction Support Broker (like us!) to develop a budget.
✅ Select Financial Intermediary (FI) services to manage your funds.
✅ Begin hiring staff and using services that align with your goals.
